10 Years of Serving Chicagoland Gala Sweepstakes

NO PURCHASE NECESSARY. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT IMPROVE YOUR CHANCES OF WINNING.

Sponsor: Polish & Slavic Federal Credit Union, 100 McGuinness Blvd., Brooklyn, NY 11222.

Sweepstakes Period: The Sweepstakes will be conducted on February 1, 2020 during the “10 Years of Serving Chicagoland Gala” event at the Copernicus Center, 5216 W Lawrence Ave., Chicago, IL 60630.

Eligibility: Sweepstakes is open to the participants of the “10 Years of Serving Chicagoland Gala” event who are at least eighteen (18) years of age and present a valid ticket to enter the drawing.  Employees, officers, volunteers and its affiliates of the Sponsor as well as immediate family (spouse, parents, siblings and children) and household members are not eligible.

Prizes:  Two (2) PSFCU Certificates of $500; Five (5) IPads – Approximate Retail Value (ARV) - $600; Three (3) 75” Samsung RU7100 4K LED TV - Approximate Retail Value (ARV) - $1,579.00.

Winner Selection: Winners will be randomly selected from among all eligible entries submitted during the Sweepstakes Period.

The odds of winning Prize depend on the total number of qualified entries submitted. It shall be the responsibility of entrant to provide contact information to Sponsor in order to be eligible to collect the prize. In addition, each winner  will be required to sign the affidavit of eligibility and the publicity release which must be received by Sponsor, within seven (7) days of being notified by Sponsor or winner will be deemed to have forfeited his/her prize.

No transfer or substitution of prizes by Prize Winner is permitted.  Sponsor is not responsible for lost, damaged, misdirected, incomplete, incorrect, or illegible ticket entries, and all such entries are void. Entries become the sole property of the Sponsor and will not be returned.

Taxes: All federal, state, and local taxes, if any, are the sole responsibility of the Prize Winner. Prize may be subject to Internal Revenue Service Form1099 reporting in what case winner will be required to submit signed W9 form to the Sponsor.

Restrictions: Employees, officers, and volunteers of the Polish & Slavic FCU and its affiliates, and their immediate family members and persons residing in the same household, are prohibited from participating in the Sweepstakes, drawings, or prizes and are not eligible to enter or win. This Sweepstakes is subject to all applicable federal, state, and local regulations.

Additional Provisions:  Prior to receipt of their prize, the winner will be required to sign an affidavit of eligibility and must grant permission to publish his/her name and likeness in Sponsor’s social media platforms, member communications and advertisements.  By participating in the Sweepstakes, participants agree to release, discharge, and hold Polish & Slavic Federal Credit Union harmless from any liability, claims, losses, or damages arising out of or relating to their participation in the Sweepstakes or the acceptance, use, misuse, or possession of any prize received in this Sweepstakes.

BY ENTERING YOU AGREE TO RELEASE AND HOLD HARMLESS POLISH & SLAVIC FEDERAL CREDIT UNION AND ITS SUBSIDIARIES, AFFILIATES, ADVERTISING AND PROMOTION AGENCIES, PARTNERS, REPRESENTATIVES, AGENTS, SUCCESSORS, ASSIGNS, EMPLOYEES, OFFICERS AND DIRECTORS FROM ANY LIABILITY, ILLNESS, INJURY, DEATH, LOSS, LITIGATION, CLAIM OR DAMAGE THAT MAY OCCUR, DIRECTLY OR INDIRECTLY, WHETHER CAUSED BY NEGLIGENCE OR NOT, FROM (I) SUCH ENTRANT'S PARTICIPATION IN THE SWEEPSTAKES AND/OR HIS/HER ACCEPTANCE, POSSESSION, USE, OR MISUSE OF ANY PRIZE OR ANY PORTION THEREOF, (II) TECHNICAL FAILURES OF ANY KIND, INCLUDING BUT NOT LIMITED TO THE MALFUNCTIONING OF ANY COMPUTER, CABLE, NETWORK, HARDWARE OR SOFTWARE, (III) UNAUTHORIZED HUMAN INTERVENTION IN ANY PART OF THE ENTRY PROCESS; (IV) ELECTRONIC OR HUMAN ERROR WHICH MAY OCCUR IN THE ADMINISTRATION OF THE PROMOTION OR THE PROCESSING OF ENTRIES. PSFCU AND ITS SUBSIDIARIES, AFFILIATES, ADVERTISING AND PROMOTION AGENCIES DO NOT MAKE ANY WARRANTIES, EXPRESS OR IMPLIED, AS TO THE CONDITION, FITNESS OR MERCHANTABILITY OF THE PRIZES. PSFCU IS RESPONSIBLE SOLELY FOR PROVIDING THE PRIZES TO THE WINNERS. ANY AND ALL WARRANTIES AND GUARANTEES ARE SUBJECT TO THE RESPECTIVE PRIZE MANUFACTURER'S TERMS.

ARBITRATION/CHOICE OF LAW: Sweepstakes shall be governed by the laws of the State of New York. Any and all legal actions or claims arising in connection with this sweepstakes must be brought in a court of competent jurisdiction within the United States. By participating in this sweepstakes, each entrant accepts the conditions stated in these Official Rules, agrees to be bound by the decisions of the judges and warrants that he/she is eligible to participate in this sweepstakes.
